I hadn't thought about the two heads or no head thing. In fact, that "explains" the Romana regeneration -- it is obviously got a funny side to it, but it is pretty clear that she's picking something to 'work' with the Fourth Doctor.
The Romana regeneration is very interesting...because she does change skin color (http://en.wikipedia.org/wiki/Image:Romanaregenerate.jpg), after all.
And after all, in the TV Movie the Doctor says that one can even change species as part of a regeneration -- and if that's possible, changing gender is, well, nothing at all.
For me, the interesting thing would be how that might change some of the other elements -- would a female Doctor have a male companion instead? What sort of dramatic impact would it *really* have, especially for what would still be intended as a mass-audience television show.
